Immunization Records
You are required to turn in a copy of your immunization records by using one of the options listed below.
Email a copy to alyssam@fortscott.edu
Fax them to (620)223-4927
Mail them to FSCC Housing Office, 2108 S. Horton, Fort Scott, KS 66701.
Where do I get my records from?
Parents and Guardians often have these records so check there first! After that you can contact your high school, doctor, or check your state’s IIS!
Why is this a requirement?
We require students to submit immunization records as a proactive measure to safeguard the health and well-being of our campus community. This requirement helps us to ensure the health and safety of the campus community by preventing the spread of contagious diseases.
